Community Dental Home Worker

Department: Glendale Medical
Location: Glendale, CA


The Community Dental Home Coordinator position is established to increase access to dental care for children ages 0-5 and perinatal patients. The CDHC duties include dental home referral coordination, parental/family education. The CDHC facilitates referral to CCHCís Dental clinics and works closely with Medical and Dental Providers, support staff, Dental Manager, Parents and children. The CDHC is to increase parentís awareness in oral health, oral disease, and improve oral health outcomes for the children 0-5 and Perinatal patients.


  • Assure access to dental care for children, ages 0-5 and perinatal patients, using case       management style approach, which includes but not limited to managing up patientís goals and referrals and assists in patient activities
  • Participates outreach activities at Health Fair and similar community events.
  • Provides oral health presentations, along with potential care options, explaining the benefits of good oral hygiene to patients and their families.
  • Schedules and confirms appointments; perform reminder calls.
  • Performs cares risk assessment, self-management goals and preventive services as needed.
  • Prepare handout materials, dental supplies, and recording forms necessary for services visits
  • Comply with OSHA, HIPAA, and State Dental Regulations at all times.
  • Schedule follow-up visits and conduct follow-up visits with parents and children based on individual child need.
  • Maintains current knowledge of oral health as well as different type of dental benefits.
  • Maintains relationships with referral sources and Medical and Dental Department
  • Maintains all Dental Department areas in compliance with Dental department directives and protocols as well as center policies and procedures relative to infection control, exposure control and safety issues.
  • Maintains a list of all Dental Department patients, monitors patient flow, and assists the Dental director, dentist in assuring that all patient records and documents are properly and accurately completed and filed. .
  • Receives and places necessary telephone calls consistent with professional matters, clinic business and patient of the dental department.
  • Participates in appropriate health promotion / disease prevention activities, both on site and off site as required.
  • As directed by a supervisor, performs other related and/or necessary tasks to achieved organizational and programmatic goals and objectives.


  • High school diploma or GED certification.
  • Certified Dental Assistant preferred.
  • At least one year experience in dental office setting.

  • Ability to independently seek out resources and work collaboratively.
  • Ability to read, understand and follow oral and written instructions.

  • Demonstrates a willingness and ability to work under supervision.
  • Ability to develop and maintain good working relationships with staff.
  • Ability to use computer and learn new software programs.
  • Excellent interpersonal skills reflecting clarity and diplomacy and the ability to communicate accurately and effectively with all levels of staff and management.
  • Demonstrates ability to work in a regulatory climate that includes oversight of state and federal entities, payer contracts etc.
  • Possesses ability to communicate effectively, both verbally and in writing.
  • Possesses genuine respect for others and acceptance of their individual social and cultural traits.
  • Proficient knowledge of Microsoft Outlook.
  • Able to travel and attend professional meetings, conferences, trainings and clinic sites.
  • Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams.
  • Knowledge of Medical Office Marketing.
  • Knowledge of the interrelationship of both people and functions within the Medical Office; includes knowledge of informal Medical Office goals, standards, policies and procedures. This knowledge presumes there may be limited knowledge of other Medical Offices within the organization.
  • Performs other related duties as assigned.


Position requires prolonged sitting at a computer, some bending, lifting, stooping and stretching. Good eye-hand coordination and manual dexterity sufficient to operate a computer keyboard, photocopy machine, telephone, and other office equipment is also required. Employee must have normal range of hearing and eyesight.
